Project Horizon delivers coordinated, person‑centred support to address the underlying factors driving repeat offending. The project aims to reduce reoffending by addressing the underlying social, economic, and health‑related factors that contribute to cyclical criminal behaviour.

* Housing and homelessness support - Helping individuals who may be homeless or need support
* Addictions support - Connecting people with substance misuse services
* Support with offending behaviours - Providing structured interventions to address criminogenic needs
* Healthcare support - Facilitating access to physical and mental health services
* Benefits and welfare support.

Who can use this service?
This service aims to provide support to vulnerable offenders in Northumberland who present with complex needs and demonstrate entrenched patterns of repeat offending.
How are services delivered?
Face to Face, Direct 1-2-1 Casework, multi agency approach.
