Cramlington and Seaton Valley PCN - Social Prescribing Link Workers
Social Prescribing Service for adults age 18 upwards. We accept referrals from all services including social workers, the police, care workers as well as pharmacists and other primary care professionals. We support patients with accessing local community-based services and groups to support their health, wellbeing and independence. We also accept self-referrals.

Who can use this service?
Anyone 18 years and over can use the service as long as they are registered with a GP surgery in Cramlington and Seaton Valley.
